INSURANCE AND PAYMENT INFORMATION:
HEALTH INSURANCE:
For health insurance plan payments,
please note the following:
1.
Bring your health insurance plan
card or other identifying information.
2.
Co-payment for the visit must be paid at the time of service and will
be accepted by cash, check, or credit card.
Metrowest Allergy accepts Visa, Mastercard, and Discover Card.
3.
It is your responsibility to know
your plan benefits including applicable copayment/coinsurance and deductible
amounts. If unsure, please call the
member service number listed on your insurance card.
4.
If you have a managed care health
plan (HMO), you will need a written referral (or authorization) from your
primary care physician (Pediatrician, Internist or Family Practice doctor).
It is your responsibility to obtain the referral and it must be on file beforeyour
consultation with the allergist. If you are unsure if you need a referral,
please confirm with your insurance company if one is needed.
Most health insurance plans in
Massachusetts are accepted. The following is a partial list of the health
plans.

Tufts Health PPO Plans
5.
Tufts HMO Plans: If you have Tufts HMO and are unable to
locate a network provider, you can request an “Out-of-Network” referral from
your primary care physician. This
referral would need to be in place prior to making an appointment.

United Healthcare (confirm with your
insurance if Dr. is in your specific plan by calling member services number
listed on your insurance card)

Tricare

If you have any
